Case report of recurrent focal segmental glomerulosclerosis (FSGS) after kidney transplantation successfully managed with a tailored immunosuppressive regimen including low-dose tacrolimus, mycophenolate mofetil, and rituximab. Early recognition through laboratory monitoring and biopsy confirmation allowed timely intervention, resulting in remission of proteinuria and stabilization of renal function. This case highlights the value of individualized immunosuppressive strategies and emerging therapies in optimizing post-transplant FSGS outcomes.
