Case report of recurrent focal segmental glomerulosclerosis (FSGS) after kidney transplantation successfully managed with a tailored immunosuppressive regimen including low-dose tacrolimus, mycophenolate mofetil, and rituximab. Early recognition through laboratory monitoring and biopsy confirmation allowed timely intervention, resulting in remission of proteinuria and stabilization of renal function.
